Who: The University of South Dakota Center for Disabilities

What they do: The Center for Disabilities advocates for increased self-determination, independence and a satisfying quality of life for people with disabilities. Their experienced team of experts offer training, technical assistance, research and educational resources to help people with disabilities, families, health care professionals and educators.

The Center also promotes statewide and national policies that create more inclusive communities and encourages South Dakota s with disabilities to pursue active, self-determined lives.

The Project WITH Supports:

A Coordinating Transitions of Care training of pediatric, family practice, and adult care nurses. The training will teach nurses how to use two new tools:

  • Transition Engagement Guide (TEG)
  • DocTalk

These tools were developed collaboratively with families, I/DD patients, pediatricians, nurses and other health professionals to facilitate successful healthcare transitions.

Through the use of these tools, nurses will collaborate to gain knowledge in the area of adolescent self-determination and have elevated expectations of what young adults with significant health care needs are capable of doing in all aspects of their lives. The Coordinating Transitions of Care training will be held on October 2nd and 3rd at the University of South Dakota.
